Case Management
The Case Management program, unique to Dane County, links older adults with services (i.e. food, housing, legal, medical, financial, social well-being etc.) to help them remain as independent as possible in their homes. Our Case Managers are devoted to coordinating, evaluating and advocating for services to meet each individual's needs and preferences.
Eligibility
Case Management services are available to all individuals 55 years old and older who are living in the City of Stoughton, Towns of Albion and Dunkirk. There is no charge for case management services. However, if you would like to give a donation for this service please direct your contribution to Cindy McGlynn, Senior Center Director.
Confidentiality
All referrals to Case Managers remain confidential. Case Managers share information only with necessary provider agencies for the purpose of providing services and only with the consigned consent of the care recipient. Persons may remain anonymous when making a referral for another person.
How to Request Case Management
Please call (608) 873-8585 and ask for a Case Manager. An appointment may be made at the Senior Center or in your home.
